Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct start-of-care visits for orthopedic and other home health patients
Develop and implement individualized physical therapy treatment plans
Collaborate with nursing and interdisciplinary team for coordinated care
Document patient progress using HomeCare HomeBase (HCHB) EMR
Travel within Tehama County to provide in-home therapy services

Qualifications

Active Physical Therapy license in California
BLS certification required
Home health experience is a must; less than 2 years considered if you have the right skills
Comfortable driving approximately 40 miles per day within service area

Knowledge & Skills

Expertise in orthopedic and geriatric care
Familiarity with CHF and COPD management
Proficiency with HomeCare HomeBase (HCHB) EMR system
Strong clinical assessment and patient education skills
