Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Provide compassionate, one-on-one physical therapy in patients' homes
Develop and implement personalized treatment plans to restore mobility and independence
Collaborate with physicians, caregivers, and interdisciplinary team members
Educate patients and families on exercises, safety, and fall prevention
Document patient progress accurately and timely in accordance with regulatory standards

Qualifications

Active California Physical Therapist license (required)
Current BLS certification (required)
Experience in home health or outpatient setting preferred
Valid driver's license and reliable transportation for home visits
Ability to travel up to 25% within service area

Knowledge & Skills

Strong clinical reasoning and assessment skills
Excellent communication and patient education abilities
Knowledge of home health regulations and Medicare guidelines
Proficient with electronic medical records (EMR) systems
Time management and organizational skills for autonomous work
