Roles & Responsibilities

• Deliver direct social work case management services to PACE participants, ensuring their well-being and access to resources.
• Conduct participant screenings, assessments, and ongoing case management to address social, emotional, and environmental needs.
• Provide in-home assessments through home visits, offering personalized support in the participant's own environment.
• Collaborate with interdisciplinary teams to develop and implement care plans that promote participant independence and quality of life.
• Maintain accurate documentation and comply with all policies, procedures, and licensing requirements.
• Travel frequently within San Diego County to meet participants where they are.

Qualifications

• Master's Degree from an accredited School of Social Work is required.
• Valid California driver's license and insured vehicle are essential.
• Bilingual in English and Spanish is highly preferred to serve our diverse community.
• Experience in case management, counseling, or home health settings is a plus.

Education

• Master's Degree in Social Work from an accredited program
