Shift Type: Monday-Friday or Tuesday-Saturday, 8-hour shifts; flexible scheduling

Hours per Week: 40

Candidate Pay Rate: $77 per hour

Roles & Responsibilities

Provide skilled nursing care to patients in their homes
Conduct comprehensive assessments including start of care, recertification, and resumption of care
Manage a caseload of approximately 5 visits per day
Educate patients and families on disease management and treatment plans
Monitor and manage patients with CHF, COPD, wounds, wound vacs, and IV-PICC lines
Coordinate with interdisciplinary team including home health aides and therapists

Qualifications

Valid RN license in California
BLS certification required
Valid driver's license and auto insurance
Experience in home health preferred
Familiarity with HCHB (HomeCare HomeBase) is a plus

Knowledge & Skills

Strong clinical assessment skills
Excellent communication and patient education abilities
Ability to work independently and manage time effectively
Knowledge of Medicare home health regulations
