Roles & Responsibilities

Conduct physical therapy assessments and interventions
Develop and implement treatment plans for patients
Collaborate with nursing staff and other healthcare professionals
Perform SOC visits as needed
Utilize EMR system for documentation

Qualifications

2 years of experience in Physical Therapy (will consider less with Home Health experience)
BLS certification required
Experience with Home Health and Physical Therapy

Education

Physical Therapy degree

Knowledge & Skills

Physical Therapy techniques and best practices
Home Health care principles
EMR system (HCHB - HomeCare HomeBase)
Communication and interpersonal skills
