Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Conduct physical therapy assessments and treatments in home health settings
- Develop and implement patient care plans
- Collaborate with healthcare teams to ensure comprehensive care
- Perform start of care, recerts, and resumption of care visits
- Complete daily visits and achieve productivity goals
Qualifications
- Current ND Physical Therapy License
- BLS certification
- Home Health experience preferred
- OASIS training preferred
- Valid Driver's License and Auto Insurance
Education
- Physical Therapy degree from an accredited program
Knowledge & Skills
- Physical therapy techniques and best practices
- Home health and hospice care principles
- OASIS data collection and reporting
- Effective communication and interpersonal skills
