Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless patient care transitions to optimize outcomes
Master complex discharge planning for safe patient returns home
Collaborate with multidisciplinary teams to navigate care pathways
Utilize EMR systems like Meditech and Midas for efficient documentation
Advocate fiercely for patients ensuring they receive the best possible care

Qualifications

Active RN license in good standing
Minimum 1 year of case management experience
Proficiency in Midas and Meditech systems required
Experience with complex discharge planning preferred

Education

RN degree from accredited nursing program

Knowledge & Skills

Expertise in RN case management
Strong skills in Meditech and Midas EMR platforms
Ability to handle complex discharge planning
Excellent collaboration with RN and SW case managers
