Location: Louisville, Kentucky
Start Date: 06-29-2026
End Date: 09-28-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (rotating weekends, 8:00 AM - 5:00 PM), flexible 5-8 hour days, including holidays
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Conduct physical therapy evaluations and assessments
- Develop and implement treatment plans for patients
- Provide skilled physical therapy interventions
- Collaborate with clinical manager or educator for support
- Perform start of care, recerts, daily visits, and other duties as needed
Qualifications
- Physical Therapy degree or certification
- BLS certification required
- Preferred experience in physical therapy, particularly in home health
- No new grads preferred, but not required

- Physical Therapy degree

- Physical therapy techniques and interventions
- Patient assessment and evaluation
- Treatment planning and implementation
- Communication and interpersonal skills
- Ability to work independently and as part of a team
