Roles & Responsibilities

Conduct physical therapy assessments and interventions for patients in home health settings
Develop and implement treatment plans for patients with various conditions, including orthopedic and geriatric patients
Collaborate with nursing staff and other healthcare professionals to ensure comprehensive care
Perform SOC visits, especially for orthopedic patients when nursing is not ordered

Qualifications

2 years of experience in Physical Therapy (will consider travelers with less experience and Home Health experience)
BLS certification required
Experience with Home Health Physical Therapy preferred
Ability to perform 5 visits per day

Education

Physical Therapy degree

Knowledge & Skills

Physical Therapy skills
Home Health experience
EMR proficiency (HCHB - HomeCare HomeBase)
Strong communication and interpersonal skills
