Contract Type: Travel Contract

Shift Type: Rotating schedule including weekends, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive home health evaluations for patients with CHF, ortho, pneumonia, and other conditions
Develop and implement individualized care plans including SOC, ROC, recertifications, evals, and daily visits
Provide skilled physical therapy interventions and educate patients and families on therapeutic exercises and safety
Document patient progress and communicate effectively with the clinical team
Collaborate with clinical manager or educator for support and guidance

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but new grads considered
Ability to work independently and manage a caseload of approximately 6.5 points per day
Comfortable with rotating weekend and holiday schedules

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation
Strong clinical skills in orthopedics, cardiopulmonary, and general medical conditions
Excellent communication and time management skills
Proficient in using provided phone and tablet for documentation and communication
