Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive Oasis assessments and start of care visits for elderly patients in a primarily rural setting
Develop and implement personalized physical therapy plans to improve mobility and function after falls or fractures
Track patient visits and time using a tablet for efficient documentation
Collaborate with a multidisciplinary team across four counties to ensure coordinated care
Provide compassionate, patient-centered therapy that makes a real difference in patients' lives

Qualifications

Active Physical Therapist license in Indiana (or compact eligibility)
Experience with Oasis assessments and home health preferred
Ability to manage a caseload equivalent to 4-6 patient visits per day
Comfortable traveling to rural and mid-sized towns in Noble, Lagrange, Steuben, and DeKalb counties
Professional attire: business casual or Hunter green scrubs with white or black top

Knowledge & Skills

Skilled in evaluating and treating orthopedic conditions, especially in geriatric populations
Proficient in electronic documentation and using tablet applications
Excellent communication and time management skills
Knowledge of home health regulations and Oasis documentation standards
