Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized treatment plans
Perform start of care, recertifications, and resumption of care visits for home health patients
Educate patients and families on therapeutic exercises and functional mobility
Collaborate with a supportive clinical team to ensure optimal patient outcomes
Document patient progress accurately and timely in electronic health records

Qualifications

Active North Dakota Physical Therapy License required
Current BLS certification needed before start
Valid driver's license and auto insurance required for home visits
Prefer at least 1 year of home health experience and OASIS training

Knowledge & Skills

Skilled in home health patient care and OASIS documentation
Ability to work independently and manage a daily caseload of 6-7 points
Excellent communication and patient education abilities
