Physical Therapist - Home Health
Location: Monticello, IA
Start Date: 05-10-2025
End Date: 12-27-2025
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Deliver compassionate home health therapy to patients in their own homes, making a real difference in their daily lives
- Develop personalized treatment plans that promote recovery and independence
- Collaborate with interdisciplinary teams to ensure top-notch patient care
- Educate patients and families on exercises and self-care techniques for lasting results
Qualifications
- Active Physical Therapy license in good standing
- BLS certification
- Experience in home health preferred
- Epic experience a plus but not required
Education
- Graduate of an accredited Physical Therapy program
Knowledge & Skills
- Strong clinical assessment and treatment skills
- Excellent patient communication and empathy
- Familiarity with home health documentation
- Epic system knowledge preferred
