  • <p>Mount View Care Center in Wausau, Wisconsin, is a 200‑bed facility, offering a wide range of care options. <strong>Nurses and CNAs will be expected to float to all units, including long term care, rehab, dementia care, or if specifically assigned, the vents unit. </strong></p>

  • Responsibilities</b>: </p>
  • <ul><li>Care, feeding, and mobility support.</li><li>Follow individualized care plans and ensure care is delivered in alignment with resident goals, preferences, rights, and safety needs.
  • ·</li><li>Assist with ambulation, transfers, and positioning using gait belts, non-mechanical aids, and mechanical lifts (sit-to-stand, full-body lifts).
  • ·</li><li>Provide restorative and range-of-motion exercises as assigned.
  • ·</li><li>Answer call lights promptly and ensure timely, compassionate response to resident needs.
  • ·</li><li>Assist residents with assistive devices, prosthetics, colostomy appliances, and adaptive equipment.
  • ·Support residents during mealtimes, including set-up, per-meal grooming, feeding assistance, documentation of intake, and ensuring nutritional safety.
  • ·</li><li>Observe and report changes in resident physical, emotional, or cognitive status to the Licensed Nurse.</li><li>Communicate effectively with residents, families, nurses, and interdisciplinary staff while maintaining professionalism and empathy.
  • ·</li><li>Provide input to care conferences and document care accurately in electronic systems.
  • ·</li><li>Maintain a clean, safe, and organized resident environment including stocking supplies, making beds, and assisting with resident personal laundry.
  • ·</li><li>Follow infection control protocols, including PPE use, hand hygiene, standard and transmission-based precautions.
  • ·</li><li>Report equipment concerns, safety hazards, or resident rights issues immediately.
  • ·Support a calm, homelike atmosphere and assist in minimizing stressors for residents. </li><li>Protect resident belongings and promptly report missing items.
  • ·</li><li>Ensure residents are free from abuse, neglect, and restraints.
  • ·Honor the residents’ right to refuse care and seek alternatives that respect autonomy.
  • ·</li><li>Accurately document ADLs, intake/output, restorative care, and other assigned tasks in the electronic charting system prior to end of shift.
  • ·</li><li>Participate in onboarding and training of new staff members.
  • </li><li>Maintain awareness of unit-specific needs, including ventilator care, dementia care, and end-of-life/hospice support.
  • ·</li><li>Participate in emergency procedures, including identifying Dr. Codes, Security Alerts, and responding appropriately.
  • ·</li><li>Assist with dining room setup, social engagement, and promotion of community-based activities.</li></ul>
