Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Coordinate patient care plans to ensure seamless transitions and optimal outcomes
Advocate fiercely for patients navigating complex healthcare systems
Collaborate with multidisciplinary teams to deliver compassionate, high-quality case management
Assess needs and connect patients with vital resources for recovery and wellness

Qualifications

Active RN license in good standing
Minimum 1 year of recent case management or related nursing experience
BLS certification preferred
Strong passion for patient advocacy and care coordination

Education

Associate or Bachelor's degree in Nursing (ADN/BSN)

Knowledge & Skills

Expertise in case management principles and utilization review
Excellent communication and interpersonal skills
Proficiency in electronic health records and care planning tools
Ability to thrive in fast-paced hospital environments
