Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate psychosocial assessments and interventions to patients and families
Coordinate care and discharge planning to ensure smooth transitions
Collaborate with interdisciplinary teams to address patient needs
Conduct 5150 evaluations and coordinate involuntary holds when necessary
Advocate for patients' rights and access to community resources
Document clinical interventions accurately and timely

Qualifications

Licensed Clinical Social Worker (LCSW) in California
Current BLS certification
5150 certification in California (completed training and passed exam with 80%)
Prior experience in a California Dignity Health facility is required
Not employed by CommonSpirit/CHI/Dignity within the past year

Education

Master's degree in Social Work from an accredited program

Knowledge & Skills

Strong assessment and crisis intervention skills
Knowledge of psychiatric holds and involuntary evaluation processes
Excellent communication and collaboration abilities
Familiarity with healthcare systems and community resources
Proficient in electronic health records
