Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, with rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists for the Palliative Care team
Review patient charts and assess individual needs
Facilitate goals of care discussions with patients and families
Provide education on disease process, advance directives, and POLSTs
Hold family meetings and offer end-of-life support
Attend daily rounds and collaborate with physicians and NPs

Qualifications

2-5 years of recent experience in palliative and/or hospice setting
Strong acute care, clinical thinking, and communication skills
BLS certification required; ELNEC preferred
ICU or oncology experience is a plus
Flexibility, organizational skills, and a desire to serve others

Knowledge & Skills

Palliative care principles and end-of-life support
Acute care assessment and critical thinking
Electronic Medical Record (Cerner) proficiency
Strong interpersonal and communication skills
Ability to work in a faith-based, collaborative environment
