Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; rotating weekends (at least one full weekend per month)

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Conduct comprehensive initial assessments, utilization review, and discharge planning for a caseload of 15-20 patients daily
Collaborate with interdisciplinary teams to coordinate care across all hospital areas (no outpatient or home care)
Apply Interqual criteria for utilization management and ensure appropriate resource utilization
Work with diverse patient populations including uninsured and homeless individuals
Document all care coordination activities in Cerner EMR
Participate in rotating weekend coverage (at least one full weekend per month)

Qualifications

Current and active California RN license
BLS certification from American Heart Association
Prior recent experience at a Dignity Health/CommonSpirit facility in California (required)
Minimum 1 year of case management experience, preferably with utilization review and discharge planning
Experience with Interqual criteria and Cerner EMR preferred
Ability to work rotating weekends and adapt to reprioritization throughout the day

Knowledge & Skills

Case management, utilization review, and discharge planning
Care coordination across multiple payers and populations
Effective communication and teamwork
Ability to reprioritize in a fast-paced environment
Proficiency with Interqual and Cerner (preferred)
