Roles & Responsibilities
- Conduct physical therapy sessions for patients in home health settings
- Perform SOC visits, especially for orthopedic patients
- Utilize EMR system (HCHB - HomeCare HomeBase) via provided tablet
- Manage a caseload of approximately 5 visits per day
- Travel within Tehama County for patient visits (approximately 40 miles per day)
Qualifications
- Physical Therapist licensure
- Home Health experience (at least 2 years, but will consider less with experience)
- BLS certification
Education
- Physical Therapy degree
Knowledge & Skills
- Physical therapy skills, especially in home health and orthopedic settings
- Strong communication and patient management skills
- Ability to work independently with minimal supervision
- Familiarity with EMR systems (HCHB - HomeCare HomeBase)
