Shift Type: Day shift, Monday-Friday, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized treatment plans
Provide therapeutic interventions to improve mobility, strength, and functional independence
Collaborate with physicians, nurses, and other healthcare professionals for coordinated care
Document patient progress in HCHB (HomeCare HomeBase) accurately and timely
Educate patients and families on home exercise programs and safety measures
Travel to patient homes within Tehama County, covering an average of 40 miles per day

Qualifications

Active Physical Therapist (PT) license in California
BLS certification required
At least 1 year of Home Health experience (candidates with less than 2 years welcome to apply)
Experience with HCHB (HomeCare HomeBase) strongly preferred
Strong clinical skills in orthopedic and geriatric patient populations

Knowledge & Skills

Home Health therapy protocols and regulations
Orthopedic and geriatric patient care
HCHB electronic medical record proficiency
Time management and self-scheduling
Compassionate communication with patients and families
