Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and create customized care plans
Guide patients and families through exercises and proper use of therapeutic equipment
Monitor progress and communicate updates to physicians and care teams
Train and supervise home health aides participating in rehabilitation programs
Document clinical findings and progress toward treatment goals
Identify additional support services patients may need and coordinate referrals

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from Physical Therapy (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient and family education on therapeutic exercises and equipment
Understanding of Medicare and Medi-Cal regulations
Ability to work independently while collaborating with interdisciplinary teams
Strong documentation and clinical reporting skills
