Roles & Responsibilities
- Assess and evaluate patients' therapeutic and functional status to create personalized care plans
- Provide hands-on physical therapy treatments that help patients regain mobility and independence
- Educate patients and families on therapeutic exercises and equipment use
- Document progress and communicate updates to physicians and care teams
- Collaborate with home health aides and caregivers to support rehabilitation goals
- Evaluate home environments to identify safety needs and optimize patient outcomes
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Valid state PT license
- Ability to work independently in home health environments

- Physical Therapy assessment and treatment techniques
- Home health care best practices
- Patient and family education strategies
- Documentation and clinical reporting
- Medicare and Medi-Cal compliance knowledge
