Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide personalized physical therapy services to home health and hospice patients
Help patients recover from surgeries, falls, and other conditions in the comfort of their homes
Develop and implement individualized treatment plans that promote independence and mobility
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Document patient progress using the HomeCare/HomeBase EMR system

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of physical therapy experience with previous home health experience preferred
Valid driver's license and auto insurance
Strong clinical skills and ability to work independently

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Familiarity with HomeCare/HomeBase EMR
Excellent communication and time management
Compassionate approach to patient-centered care
