Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for geriatric patients with multiple comorbidities including CHF and COPD
Complete approximately 5 meaningful patient visits per day using the HCHB EMR system
Collaborate with the intake RN and office manager to coordinate optimal patient care
Travel within the service area while enjoying a tablet provided for efficient documentation

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Strong assessment and clinical skills for orthopedic and geriatric populations

Knowledge & Skills

Home Health physical therapy expertise
Proficiency with HomeCare HomeBase EMR system
Excellent time management for managing daily visit schedules
Strong communication skills for patient and family education
Ability to work independently while maintaining quality care standards
