Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Complete approximately 5 meaningful patient visits each day
Document care using HomeCare HomeBase EMR on your provided tablet
Collaborate with the intake RN and manager to coordinate patient needs
Support geriatric patients managing multiple conditions like CHF and COPD
Help patients recover from joint replacements and orthopedic procedures

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home Health documentation and compliance
Orthopedic rehabilitation expertise
Geriatric patient care with multiple comorbidities
Comfortable working independently in home settings
