Roles & Responsibilities

Conduct physical therapy assessments and develop treatment plans
Provide high-quality patient care in a home health setting
Collaborate with nursing staff and other healthcare professionals
Perform SOC visits as needed, especially for ortho patients

Qualifications

Physical Therapy degree and licensure
BLS certification required
Home Health experience preferred, but not required
Ability to work independently with minimal supervision

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Physical therapy techniques and best practices
Home health care principles and guidelines
EMR systems, specifically HCHB - HomeCare HomeBase
Strong communication and interpersonal skills
