Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform comprehensive start of care, recertifications, and evaluations for home health patients
Deliver personalized daily visits to patients recovering from CHF, orthopedic procedures, and pneumonia
Collaborate with clinical managers to ensure exceptional patient outcomes
Document patient progress and adjust care plans as needed
Provide compassionate care while traveling throughout the Louisville area

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but not required
Strong communication and time management skills

Knowledge & Skills

Home health physical therapy practices
Patient assessment and care planning
Electronic documentation systems
Common diagnoses including CHF, orthopedic conditions, and pneumonia
