Roles & Responsibilities

Deliver personalized physical therapy care directly in patients' homes
Create customized treatment plans that help patients regain mobility and independence
Collaborate with interdisciplinary teams to ensure seamless care coordination
Educate patients and families on exercises and recovery techniques
Document progress and adjust care plans to achieve optimal outcomes

Qualifications

Current California Physical Therapist license
BLS certification
Home health experience preferred
Strong communication and interpersonal skills

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Ability to work independently in various home settings
Compassionate bedside manner
Strong documentation and time management abilities
